A Valid government-issued photo identification (must not be expired);
Hawaii Hunter’s Education card issued within the last 4 years for long-gun permits; or
or if acquiring a handgun, a handgun safety course taken within the last 4 years as described in HRS §134-2 (as amended by SB1230).This training must be evidenced by an affidavit that is compliant with (HRS §134-2 (g)(4)) (as amended by SB1230). Your firearms trainer must be preapproved by the Chief of Police and will be familiar with the requirements of this affidavit.

- For naturalized citizens, LPRs or COFA migrants:
  - If born outside of the United States, proof of U.S citizenship, (valid US Passport, original Naturalization Certificate, or Consular Report of Birth Abroad); or
  - Valid Lawful Permanent Resident Card - Form I-551 (Green Card) if applicable; or
  - COFA documents.
A signed Mental Health waiver - Authorization for Use or Disclosure of Protected Health Information (PHI)
If you are under 25 years of age, you are required to complete, sign and submit the Authorization for Release of Information of Juvenile Records.
If you are applying for the first time or have not renewed a permit after December 4, 2016, you will also be required to provide the Fingerprint Consent & Notification form.
